Description

This program supports the development of effective state chronic disease programs to strengthen public health science and practice by addressing crosscutting functions, domains, settings, risk factors and diseases. Long-term outcomes include: increased availability and accessibility of learning opportunities addressing leadership and chronic disease prevention approaches; improvements in processes and systems for public health planning, implementation and evaluation; increased implementation of evidence-based public health approaches; and increased dissemination of public health tools and resources.

Eligible Applicants

The intent of this funding is to support the development of effective state chronic disease programs by disseminating effective chronic disease prevention and health promotion approaches to strengthen public health science and practice. Funding eligibility is limited to national, public non-profit organizations, that work with state and territorial health departments and whose primary focus is chronic disease prevention and health promotion.
