Description

Community Transformation Grants - To reduce death and disability from the five leading causes of death through the prevention and control these conditions and their risk factors. Recipients will select from a menu of interventions across the health and wellness spectrum, each of which can prevent or control chronic conditions. Each recipient will choose interventions that will help to achieve health equity, eliminate disparities, and improve the health of all groups. More specific goals within these broad goal areas will be provided in funding announcements National Dissemination and Support for Community Transformation Grants - To support the efforts of the CTG program by funding national non-governmental organizations with a network of community based organizations.

Types of Assistance: COOPERATIVE AGREEMENTS

Eligible Applicants

Applications may be submitted by State or local governments or their Bona Fide Agents (this includes the District of Columbia, the Commonwealth of Puerto Rico, the Virgin Islands, the Commonwealth of the Northern Marianna Islands, American Samoa, Guam, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau). Eligible applicants also include public and private nonprofit organizations, for profit organizations, small, minority, women-owned businesses, universities, colleges, research institutions, hospitals, community-based organizations, faith-based organizations, Federally recognized Indian tribal governments, Indian tribes, and Indian tribal organizations.
