This award will be a continuation of funds intended only for grantees previously awarded under National Diabetes Prevention Program (National DPP): Preventing Type 2 Diabetes Among People at High Risk CDC-RFA-DP12-1212. The award continues partnerships with the grantees under DP12-1212 that have existing networks and/or affiliate organizations in multiple states to scale, deliver, and sustain the National DPP. Public, private and non-profit National Organization recipients pursue strategies that scale and sustain their program delivery effort beyond small-scale demonstrations and pilots. http://www.cdc.gov/diabetes/prevention
